More and more people are realizing the
possibilities of living out their later years at home, even when the need for
chronic care arises. Advances in the long term care field are making it a
reality for many Americans. This plan can cover your actual expenses (up to the
Daily Maximum amount you choose) for benefits and services provided to you in
your Home or Community, when you qualify.
Benefits For Services Provided in Your Home or Community:

Long Term Care Services |
To help improve and maintain your quality of life, the
policy will help pay for the costs of services provided by a licensed
professional nurse, or a licensed physical, speech, respiratory, or
occupational therapist.

Home Health Agency Services |
As you get older you may need help with everyday activities
like bathing and dressing. This policy will help pay the costs for those
services and personal self care activities that are referred to as
"Activities of Daily Living." These activities include: ambulation,
bathing, toileting, continence, dressing, eating, and transferring (such as
moving from a bed, chair or wheelchair). The coverage may also provide
invaluable assistance with such things as doing your laundry, cooking,
cleaning and shopping.

Respite Care |
This plan can help pay for services that provide an
alternative care giver to assist you, allowing your primary care giver the
opportunity to take a break and enjoy some well deserved rest. (This benefit
is limited to 14 days per calendar year.)

Adult Day Care |
This policy will help pay for the costs of a program that
offers social and health related services during the day in a community group
setting. The purpose of this program is to provide support to you if you are
frail, impaired or disabled and can benefit from care outside the home during
the day, while your primary care giver provides support to you at home during
the night.

Hospice Services |
If you are confined in a Hospice Care Facility, this plan
will help pay for Hospice Care Services that help alleviate the physical,
emotional, social and spiritual discomforts you may experience due to a
terminal illness. These services provide spiritual and psychological support
for your primary care giver and your immediate family as well.
