Facility Policy
This plan is designed to
pay the fixed dollar amount of the Maximum Daily Benefit you choose for a
qualified confinement in a Nursing Facility or Residential Care Facility for the
Elderly that provides skilled, intermediate or custodial care on a 24 hour
basis. Once you begin your confinement, premiums will be waived after 60 days.
Benefits For Services Provided in a Nursing Facility or a
Residential Care Facility for the Elderly Include:

Nursing Facility Confinement/Residential Care Facility
for the Elderly Benefit |
If you are confined in a Nursing Facility or Residential
Care Facility for the Elderly, we will pay you the Daily Benefit amount you
have chosen.

Nursing Facility Bed Reservation |
If you are receiving benefits under the policy and you
require hospitalization while confined in a Nursing Facility, we will pay a
benefit to reserve your bed in a Nursing Facility for up to 21 days per
calendar year, when you incur charges for such reservation. The benefit paid
will be equal to the Nursing Facility Confinement Daily Benefit.

Alternative Payment of Benefits |
If you are otherwise eligible for benefits and if agreed
upon by you, your physician and us, we will consider payment of your claim for
services or equipment not specifically shown in the policy, subject to the
total maximum amount payable under the policy.
