Long
Term Care
Policy Details and Specifications
Daily Benefit Amounts
Tailor the amount of benefits you would like to receive.
Choose from $50 per day up to $250 per day. This policy reimburses your actual
expenses incurred for Home Care and pays you the full Daily Benefit amount you
have chosen for Nursing Facility Confinement. You may choose between 50% or
100% of the Nursing Facility Confinement benefits for the Home Care coverage
(minimum Daily Benefit for Home Care is $50). Before deciding on how much
Daily Benefit you want, it would be wise to research the costs of different
facilities in your area.
Elimination Periods
Choose the waiting period to fit your particular needs. This
waiting period is the number of days you must be confined in a Nursing
Facility or a Residential Care Facility for the Elderly, the number of days
you must receive Home Care, or any combination of these before benefits become
payable under this policy. You need only satisfy the elimination period once
during the life of the policy. Your options include: 0 Days, 30 Days or 90
Days.
Plan Maximums
Choose the amount of long term care benefits you would like
to receive from us. You may choose lifetime (unlimited) benefits or you may
choose a benefit amount equal to your daily benefit, multiplied by 1825 or
1095. For example, if you select a $100 daily benefit amount and a multiplier
of 1825 is used, the total maximum amount payable under the policy will be
$182,500. A $100 daily benefit selection and a multiplier of 1095 will result
in a $109,500 total maximum amount payable under the policy.
Premium Payment Options
Select when and how you would like to make your premium
payments: level premium payments can be made monthly (by automatic bank
draft), quarterly, semiannually and annually. On certain plans you may also
choose our 10 Pay Option or our Single-Pay Option.
